OCIA Godparent Information form

Step 1:
Contact the parish office at (480) 838-7474. Leave a message with your name, phone number, email, and best time to reach you. Include OCIA in the subject line.

Step 2:
Complete the Adult Inquirer Information Form. Please email it to parish@holyspirit-tempe-az.org or drop it off at the parish office, attention OCIA Program or mail it back to the parish office (1800 E. Libra Dr. Tempe, AZ 85283), attention OCIA Program.

Step 3:
After your completed Adult Inquirer Information form is received, you will be contacted to schedule an interview with our pastor, and the OCIA coordinator.
 
Step 4:
For those individuals who are Catholic and wishing to receive the Sacrament of Confirmation, please complete the Godparent/Sponsor Information form as well. Follow instructions in Step 2.

Are you interested in joining the Catholic faith or perhaps learning more about it?

The Oite of Christian Initiation for Adults (OCIA) is the process through which adults (and children/teens) become full members of the Catholic Faith. 

• If you have never been baptized or, if you have been baptized in another Christian church and would like to explore the possibility of becoming part of the Catholic church, you are invited to attend.  There is absolutely no pressure to become Catholic. We want you to be confident of your decision to become Catholic.

• If you were baptized as a Catholic but never received any formation or never received the remainder of the sacraments of initiation (i.e., Confirmation/and or Eucharist) this process is for you as well.

• If you are a fully initiated Catholic (received all your sacraments), becoming a sponsor for one of our catechumens or candidates may be a great way to enrich your understanding of the faith while encouraging someone in the process as well.
